• Name and telephone number of referring physician
• Photo identification card
• Name and telephone number of your primary care physician
• Copies of all old and new MRI, CT, X-RAY films AND reports
• Copies of any EMG Nerve Conduction studies
• Date and types of all previous surgeries, if any
• List of current medication taken by patient
• Insurance policy numbers and provider names
• Bring a family member or translator if you do not speak English
